Delirium Checklist
It's important to let the healthcare professionals know if the person you are visiting seems to be getting sicker or if something just doesn't seem right. This could be a change in how they feel, look, or behave. Often, family members, partners, or friends notice these changes.
The Delirium Checklist Tool* is a questionnaire designed for carers to screen for delirium (sudden confusion) in older adults. During your visit today, have you noticed any sudden changes to your family member, partner, or friends’ usual behaviour? If they are usually non-attentive or disordered in their thinking due to conditions like dementia, and there is no change from their normal behaviour, please score them a 0.
If your family member, partner or friend scored 4 or more on the Delirium Checklist Tool, or you are concerned about them, please inform a healthcare professional (e.g. nurse or doctor) as soon as you can, for a formal assessment.
Even if your family member, partner or friend scored less than 4 please continue to use this Checklist so you can continue to support the prevention of delirium in older adults.
